REQUEST FOR STATEMENTS OF QUALIFICATIONS FOR PROFESSIONAL CONSULTING SERVICES

The Wood County Board of Developmental Disabilities is requesting statements of qualifications for professional consulting services for a new inclusive playground for the Wood Lane School located at 1921 E. Gypsy Lane Road. The Wood County Board of Developmental Disabilities intends to review and rank the submitted statements of qualifications, then enter into a contract with the most qualified firm. Three copies of the Statement of Qualifications (SOQ) should be sent to the Wood County Board of Developmental Disabilities, Attention: Scott McKeown, at 1921 East Gypsy Lane Road, Bowling Green, OH 43402, by 10/17/2025 at 4:00 pm.

 

DESCRIPTION OF PROJECT / SCOPE OF SERVICES

The Wood County Board of Developmental Disabilities intends to contract with a professional consulting firm to complete design documents and construction administration for a $1,400,000 inclusive playground. This project is expected to include the following scopes of work (please reference Exhibit A which may be acquired at www.woodcountydd.org or by email at smckeown@woodcountydd.org):


·        Removal of existing playground and necessary landscape

·        Reconfigure hardscape/softscape in accordance with Exhibit A to include the following:

o   Adventurous play area

o   Gentle play area

o   Open space

o   Flex zone

o   Sensory garden

o   External pathway

o   Internal pathway

·        Infrastructure including stormwater management if required

·        Fencing as/where required

·        Landscaping

·        Opinion of Probable cost for approved design documents

·        Permitting

·        Public Bidding Process and Documents

·        Assistance with selection/purchasing of playground equipment

REQUIREMENTS FOR STATEMENTS OF QUALIFICATIONS

The statement of qualifications should be no longer than 10 pages and contain the information requested below. Statements of qualifications of more than 10 pages will not be accepted.  Submissions will be scored based on the criteria and weighting below.

 

1. Firm Information (15 points)

·        Provide the name, office address, and a brief description of the firm and any subconsultants.

·        Include the number of years in business and any pertinent historical information

·        Include a listing of previous experience with the Wood County Board of Developmental Disabilities

2. Project Team Qualifications (20 points)

·        Include the project manager and key team members resumes.

3. Relevant Project Experience (20 points)

·        Include information on three park/playground projects the firm has completed or is currently working on.

4. References (15 points)

·        Provide three client references (Name, Title, Organization, Phone, Email) for similar projects the anticipated project team has completed or is currently working on.

5: Project-Specific Qualifications (30 points)

·        Include a one-page narrative describing the firm’s interest in the project, particular abilities and qualifications related to this project, and reasons why the firm should be selected as the consulting firm.

Sealed bids will be received by the Wood County Board of Developmental Disabilities (“Owner”), at the office of 1921 E. Gypsy Lane Road – Building B , Bowling Green, OH 43402 on the 12 th of May at 10:00am local time for the: Wood Lane School Play Space and will be publicly opened and read immediately thereafter. Each bid must contain the full names of the party or parties making the same, with an affidavit as to interested parties, and in the case of a corporation not chartered in Ohio, with a proper certificate that such corporation is authorized to do business in Ohio, and shall be accompanied by a Bid Guaranty Bond for the full amount (100%) of the Bid, in favor of the “Owner” or a certified check, cashier’s check, or letter of credit pursuant to Chapter 1305 of the Ohio Revised Code, in the amount of 10% of the Bid, payable to the “Owner”. It is agreed that the bid guaranty shall be for the benefit of the “Owner” pursuant to Section 153.54 of the Ohio Revised Code if the undersigned fails to execute the Contract in conformity with the Form of Contract incorporated in the contract documents and furnish Bond and insurance certificates within ten (10) days after the notification of the award of the Contract to the undersigned. Copies of the Contract Documents are on file and may be obtained directly from Becker Impressions , at cost, none of which will be refunded. Please note that in order to be considered as a “responsive bidder” for this project, you must purchase a set of plans and bid documents, and follow all instructions provided. Bids from contractors who are not on the Plan Holders List will be disqualified. A pre‐bid meeting will be held at 10:00am on Wednesday, May 6, 2026 at the Wood County Board of Developmental Disabilities –  Building B , 1921 E. Gypsy Lane Road, Bowling Green, OH 43402. Successful Bidders shall conform to the Ohio “Schedule of Prevailing Wages”. The bidder may access the Ohio Department of Commerce, Wage & Hour Bureau at its website for the current edition of wage rates. This Advertisement for Bids document is for use on public projects in conjunction with AIA Document A701, "Instructions to Bidders." Coordinate content with the Instructions to Bidders. Much of this Document consists of Project-specific data. Examples given in the model document text below illustrate possible document content. Use the model text to develop text for specific Project requirements. Revise "Notice to Bidders" Paragraph below to comply with Owner requirements for notification and conduct of public bidding. Retain "Printed Procurement and Contracting Documents" or "Online Procurement and Contracting Documents" Paragraph below and revise to suit Project.
