Age 14 Through Graduation

You Don't Have to Navigate This Transition Alone

Our Transition Team is committed to collaborating with the local schools, providers, agencies, individuals, and families in support of a student’s transition to adulthood. The services offered are available for youth and young adults starting at age 14. 



Students are supported while building self-advocacy and self-determination skills to develop life goals that meet their interests, preferences, and needs as they explore careers to help identify their path to employment and place in our community.

For You

A Transition Specialist or Employment Navigator will be assigned based on the needs of each individual and will be part of the Individualized Service Plan (ISP) Team. Transition Specialists work with students ages 14 through graduation and our employment navigators work with young adults graduated through age 30.

Our Transition Team May Help You With The Following:

  • Attending Individualized Education Program (IEP) meetings to assist service coordinators, schools and students with transition-related services and backwards planning.
  • Attending ISP meetings to help individuals identify and move along their path to employment.
  • Work with individuals to complete assessments and discovery tools to identify preferences, interests, needs, and services of each student.
  • Facilitating tours of adult day waiver services and links to community resources.
  • Assisting with connecting individuals to Opportunities for Ohioans with Disabilities (OOD) and Ohio Means Jobs. 
  • Plan and assist with finding meaningful employment in the community.
  • Assisting with exploring and obtaining post-secondary education options.
  • Assist with facilitating summer experiences and our monthly transition group that meets during the school year.
