SERVICE & SUPPORT ADMINISTRATION

A Service & Support Administrator (SSA) utilizes a person-centered approach to identify an individual's strengths and needs. An Individual Service Plan (ISP)is created and the SSA offers referral, linkage, and community resources to meet the needs identified in the plan. The SSA monitors those services to ensure the health & safety of the individual and that services are being provided according to the ISP.

  • INTAKE & ELIGIBILITY

    Individuals go through an application & eligibility process prior to receiving County Board Services. This includes an assessment, known as the Children’s Ohio Eligibility Determination Instrument (COEDI), for children under age 16 and the Ohio Eligibility Determination Instrument.

  • SERVICE COORDINATION

    Service coordination is a key responsibility of the Wood County Board of Developmental Disabilities.  Service & Support Administrators (SSA) help individuals and their families plan, establish outcomes, identify needs, evaluate available service options, connect with community resources and monitor services. 

  • FAMILY SUPPORT SERVICES (FSS)

    Provides supports and services to enable families to care for their family member(s) with disabilities at home. This program is available to any individual eligible for WCBDD services, who is 22 years old or younger, lives with his/her 

    family, and does not have a Home and Community Based Services (HCBS) waiver.

  • TRANSITION

    Individuals identifying outcomes for life after high school graduation can access transition and employment navigation services. A person-centered method is utilized to assist the individual in identifying their path to employment or post-secondary options. engagement, and improves quality of life.

  • RESPITE

    Provides planned, short-term limited breaks for families spanning from one night to one week at a time. Any individual aged 3 or older receiving Wood County Board of DD services is eligible. Respite can help stabilize and improve the emotional and mental well-being of both 

    families and individuals.

  • SUPPORT SPECIALISTS

    Consult, coordinate, and provide behavior support and assistance to individuals, families and Home and Community Based Service (HCBS) providers utilizing trauma informed care practices and approaches.

  • MEDICAID SPECIALISTS

    Work closely with Service and Support Administrators (SSA) to ensure waiver authorizations are complete for Home and Community Based Services (HCBS) Providers to bill for payment of services provided.

  • TRANSPORTATION

    Provides accessible transportation on evenings and weekends to individuals served plus one rider to any location. Promotes independence, community engagement, and improves quality of life.

  • HOUSING COORDINATION

    Identifies housing needs of individuals & their families. Works with landlords, housing authorities and community partners to provide safe, accessible and affordable housing 

    in Wood County. 

  • INVESTIGATIVE AGENT

    Review all unusual incident reports and identify trends & patterns that may exist. Collaborate with the ISP team to identify prevention plans to ensure the health & safety of all individuals receiving services.
