The eligibility process can feel overwhelming, but we are here to help you every step of the way. If you or a loved one could benefit from services provided by Wood County Board of Developmental Disabilities, your journey will begin with our Intake and Eligibility Department.


Once a qualifying diagnosis has been established, our team will assist you in completing the Children’s Ohio Eligibility Determination Instrument (COEDI) or the Ohio Eligibility Determination Instrument (OEDI). The assessment must result in three substantial function limitations in major life areas.

ONCE ELIGIBILITY IS DETERMINED...


AGE 0-3

A person must have at least one developmental delay determined by our Early Intervention Team or a diagnosed physical or mental condition with a high probability of resulting in a delay.


AGE 3-5

A person must have at least two developmental delays determined by a physician or qualified professional.


AGE 6-15

A person must have a diagnosis of a developmental disability documented by a qualified professional. This must be a physical or cognitive impairment other than a mental illness and must be likely to continue indefinitely. The disability must also result in substantial functional limitations in at least three of the six areas of life activity on the Children’s Ohio Eligibility Determination Instrument (COEDI) including self-care, receptive and expressive language, mobility, learning, self-direction, and capacity for independent living.


AGE 16+

A person must have a diagnosis of a developmental disability documented by a qualified professional. This must be a physical or cognitive impairment other than a mental illness and must be likely to continue indefinitely and have occurred before the age of 22. The disability must also result in substantial limitations in at least three of the seven areas of life activity on the Ohio Eligibility Determination Instrument (OEDI) including self-care, receptive and expressive language, mobility, learning, self-direction, capacity for independent living, and economic self-sufficiency.
