DETERMINING ELIGIBILITY


EARLY INTERVENTION

BIRTH TO AGE 3

A person must have at least one developmental delay determined by our Early Intervention Team or a diagnosed physical or mental condition with a high probability of resulting in a delay.


PRESCHOOL

AGE 3-5

A person must have at least two developmental delays determined by a physician or clinical professional, be eligible for special needs preschool, or a diagnosed physical or mental condition with a high probability of resulting in a delay.


CHILDHOOD

AGE 6-15

A person must have a diagnosis of a specific disability documented by a qualified professional. This must be a physical or cognitive impairment other than a mental illness and must be likely to continue indefinitely. The disability must also result in substantial functional limitations in at least three of the six areas of life activity on the Children’s Ohio Eligibility Determination Instrument (COEDI) including self-care, receptive and expressive language, mobility, learning, self-direction, and capacity for independent living.


ADULT

AGE 16+

A person must have a diagnosis of a specific disability documented by a qualified professional. This must be a physical or cognitive impairment other than a mental illness and must be likely to continue indefinitely and have occurred before the age of 22.

The disability must also result in substantial limitations in at least three of the six areas of life activity on the Ohio Eligibility Determination Instrument (OEDI) including self-care, receptive and expressive language, mobility, learning, self-direction, capacity for independent living, and economic self-sufficiency.
