We are seeking an energetic and dynamic Service Coordinator (Case Manager/Social Worker). The Wood County Board of Developmental Disabilities (WCBDD) coordinates services for over 1,000 people with developmental disabilities in Wood County. Currently employs approximately 140 dedicated employees. Our mission is to support, empower, and inspire all people. WCBDD is looking for an experienced and dynamic Service Coordinator. The ideal candidate will be a highly self-motivated problem solver, who is a team player with time management and leadership skills. The Service Coordinator will manage a caseload of individuals, be responsible for writing the individual service plan and monitor the services to ensure health and safety.

Essential Duties
  • Schedule and facilitate an Individual Service Plan (ISP)/ Individual Family Service Plan (IFSP) meetings using a person-centered philosophy.
  • Develop and amend as needed the ISP/IFSP, including service authorizations for individuals based on assessed need and using the Interdisciplinary Team process.
  • Be responsible for implementing, documenting and monitoring ISP data.
  • Assess, coordinate and ensure implementation of services consistent with individual needs which are consented to in accordance with the Individual Service Plan.
  • Provide direction of all services and behavioral programs.
  • Ensure individual/guardian choice in the selection of providers.
  • Collaborate with local school districts and Early Intervention providers.
  • Provide information and assistance in securing services for eligible children and adults into educational, vocational, housing, and other community-based service.
  • Document all service and support activities through Targeted Case Management.
  • Share 24-hour on-call responsibilities with other Service Coordinators in crisis intervention and stabilization.

 

Application Requirements

Must have achieved a Bachelor's Degree from a college or university in Psychology, Social Work, Special Education or other related human service area. A minimum of 2 years experience working directly with persons with DD as a member of their team is preferred. Must obtain/maintain certification as mandated by Ohio Law, Rules of the Director of DD and/or Rules of the Director of the State Board of Education. Must submit a resume and letter of intent.

Pay Rate

$45,406.40 - $46,966.40

Maintains constant surveillance of patrons in the facility; acts immediately and appropriately to secure safety of patrons in the events of emergency. Provide emergency care and treatment as required until the arrival of emergency medical services. Presents professional appearance and attitude at all times, and maintains a high standard of customer service.

Must have current, valid lifeguard certification issued from American Red Cross, YMCA, Ellis & Associates or Boy Scouts of America. Skill in the application of lifeguarding surveillance and rescue techniques. Must have the ability to react calmly and effectively in emergency situations.

Minimum Qualification Standards:

  1. Must be able to move individuals who are receiving services including adults and children in a safe manner. Must be able to lift, carry and move office supplies, materials, and equipment required for day-to-day operations.
  2. If required by the position you must maintain a current driver’s license in your residing state for the purpose of transporting individuals who are receiving services and/or operation of a Board owned vehicle.

 

Conditions of Employment

  1. Successful completion of general orientation.
  2. Receipt of seven acceptable background required by ORC 3319.39 & OAC 5123:2-2-02.
  3. Must obtain and maintain any training, licenses, registration, or certification mandated by the Board and the State of Ohio.
  4. Must have a medical examination and a one-step PPD test to determine fitness for duty prior to providing direct care service or within 30 days of employment whichever comes first.
  5. Possess/maintain an acceptable motor vehicle record (if required for position) per board guidelines. NOTE: Use of personal vehicle for transporting consumers requires proof of liability insurance.
  6. Receipt of acceptable criminal background report from Bureau of Criminal Identification (BCI) and county sheriff ‘s office of permanent address before starting position as required by ORC 3319.39 & OAC 5123:2-2-02.
